
This fall University Health Services is
implementing the NCHA Survey on our campus. If you receive an
invitation to participate, we hope that you will do so.
1,800 students will be randomly selected to participate in the
survey. The data from this survey will help University
Health Services to improve our programs and services by
identifying the most important health concerns of our students.
This is a web based survey. Data transmission is
encrypted and firewall securities are in place. After you submit
the survey to secure server, you will receive a message thanking
you for taking the NCHA-Web. The final survey responses will be
housed at ACHA. Again, the version of the data set that is
forwarded to your institution will not include personal
identification such as e-mail addresses or ID numbers. To

To encourage participation in the survey
University Health Services will enter the name of each student
who completes a survey into a raffle. We will award the
following prizes to three students: a $150.00 iTunes card, a one month
parking pass and a $50 gift card to the UMB Bookstore.
